{"id":4891,"date":"2021-05-17T11:38:00","date_gmt":"2021-05-17T11:38:00","guid":{"rendered":"https:\/\/edwardseducation.com\/edwardsmasters\/?p=4891"},"modified":"2023-04-23T11:48:11","modified_gmt":"2023-04-23T11:48:11","slug":"spavalve","status":"publish","type":"post","link":"https:\/\/dev.edwardseducation.com\/edwardsmasters\/spavalve\/","title":{"rendered":"Biological versus mechanical prostheses for aortic valve replacement"},"content":{"rendered":"\n<p class=\"has-text-color has-light-grey-color\">Rodr\u00edguez-Caulo, Emiliano A., <em>et al., Journal of Thoracic and Cardiovascular Surgery <\/em>(2021) <a href=\"https:\/\/doi.org\/10.1016\/j.jtcvs.2021.01.118\"><strong>DOI:10.1016\/j.jtcvs.2021.01.118<\/strong><\/a><\/p>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"has-brand-primary-color has-text-color wp-block-heading\"><strong>Aim<\/strong><\/h3>\n\n\n\n<p> Evaluate long-term survival and major adverse events in patients aged 50 to 65 years with a primary isolated AVR, with a bioprosthesis or mechanical valve, because of severe AS<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"has-brand-primary-color has-text-color wp-block-heading\"><strong>Methods<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\"><li>National\nobservational study with all consecutive patients aged 50-65 who underwent AVR\nbecause of severe isolated AS between 2000 and 2018 in 27 hospitals in Spain<\/li><li>In\ntotal, 5215 patients were included in the study (21% were bioprostheses)<\/li><li>Among\nbioprostheses, 46.8% were Carpentier-Edwards, 19.4% Mitroflow, 4.2% Mosaic and\n4.1% Trifecta<\/li><li>2:1\npropensity matching analysis (1822 Mech and 911 Bio) with a mean follow-up of\n8.1 years<\/li><\/ul>\n\n\n\n<p><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"has-brand-primary-color has-text-color wp-block-heading\">Results<\/h3>\n<\/div><\/div>\n\n\n\n<figure class=\"wp-block-image size-large\"><img fetchpriority=\"high\" decoding=\"async\" width=\"484\" height=\"277\" src=\"https:\/\/edwardseducation.com\/edwardsmasters\/wp-content\/uploads\/sites\/2\/2021\/04\/Spavalve.png\" alt=\"\" class=\"wp-image-4892\" srcset=\"https:\/\/dev.edwardseducation.com\/edwardsmasters\/wp-content\/uploads\/sites\/2\/2021\/04\/Spavalve.png 484w, https:\/\/dev.edwardseducation.com\/edwardsmasters\/wp-content\/uploads\/sites\/2\/2021\/04\/Spavalve-300x172.png 300w\" sizes=\"(max-width: 484px) 100vw, 484px\" \/><\/figure>\n\n\n\n<ul class=\"wp-block-list\"><li>In the matched cohort, long-term survival differences were not observed (cf. figure)<\/li><li>Overall survival after Mech valves at 5, 10, 15 and 18 years of follow-up were 91%, 83%, 73% and 62% <em>vs. <\/em>89%, 81%, 74% and 72% for bioprostheses, respectively<\/li><li>Minor differences in survival were found in patients aged 50-55 years (<em>P <\/em>= 0.11) <\/li><li>No significant differences in the composite of stroke, bleeding and reintervention (<em>P <\/em>= 0.484)  <\/li><li>Major bleeding (<em>P <\/em>= 0.002) and strokes (<em>P <\/em>= 0.095) had a higher frequency in the Mech valve group<\/li><li>Rates of reoperation were significantly higher in the bioprosthesis valve group (<em>P <\/em>&lt; 0.001)&nbsp; <\/li><li>In a 1:1 propensity matching, the risk of reintervention in the Bio group decreased for patients who underwent AVR between 2009 and 2018 compared with 2000 to 2008<\/li><\/ul>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"has-brand-primary-color has-text-color wp-block-heading\"><strong>Conclusions<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\"><li>Bioprostheses\nseem a reasonable choice for patients aged 50 to 65 years in Spain,\nparticularly for those older than 55 years, because of the long-term survival\nand the lower-risk related, especially, to major bleeding compared with\nmechanical prostheses<\/li><li>However,\nthe higher risk of reoperation with bioprostheses should be considered and\ndiscussed with the patient to make the best-informed decision<\/li><\/ul>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<h3 class=\"has-brand-primary-color has-text-color wp-block-heading\">Key\ntalking points<\/h3>\n<\/div><\/div>\n\n\n\n<p><strong>Are these outcomes aligned with guideline recommendations? <\/strong><em>\u2013 Current ESC\/EACTS guidelines consider mechanical and bioprosthetic valves as reasonable in patients aged 60 to 65 years, whereas AHA\/ACC guidelines consider both reasonable in 50 to 65 y.o. patients. Therefore, findings from this national observational study with patients that underwent isolated AVR seem to be more aligned with the latest American guidelines.<\/em><\/p>\n\n\n\n<p><strong>Why did the risk of reintervention decrease in patients who underwent Bio AVR between 2009 and 2018? <\/strong>\u2013 <em>The authors point to improved outcomes after AVR for both mech and bio prostheses after 2009. In the unadjusted cohort, there was a significant decrease in stroke, major bleeding and reintervention. In the bio group, the rate of reoperation because of SVD diminished from 7.6% to 4% after 2009, which is related to the durability of bioprostheses.
